To Whom it May Concern:

I am looking for the GuideStar report for ColumbiaCare. www.columbiacare.org When I perform an organization search, it returns no results. However, I looked up the charity in ProPublica's NonProfit Explorer and it returned this information:

COLUMBIACARE SERVICES INC

3587 HEATHROW WAY, MEDFORD, OR 97504-4004 | TAX-EXEMPT SINCE OCT. 2003

EIN: 81-0609831

https://projects.propublica.org/nonprofits/organizations/810609831

When I look up the EIN number in GuideStar I am directed to Oregon Regional Behavioral Services. A click on their website from the QuickFacts www.orbsinc.org, redirects to the ColumbiaCare website. A different search in the GuideStar source in the Lexis Advance database has a GuideStar report for ColumbiaCare dated October 25, 2017.

Do you know why there is a GuideStar report in Lexis Advance and not one on the GuideStar website? Do you know why the EIN listed in the NonProfit Explorer database shows up with something very similar in the GuideStar database? A comparison of the figure from 2015 is the same, but the address and some of the board members are different.

Please let me know what you find out regarding these discrepancies.

Thanks!

Beth V.
